The Gilles fan flap was once a popular way to reconstruct lip defects, and despite its sortcomings diagrams of it can be found in many text books.

The unfavorable scar configuration however makes this flap a distant second option to other reconstructions.

The same rounding of commissures occurs as with the Karapandzic, but the angular scar, is much more noticeable and unnatural.
